Compensation

$37.50 -$39.26/hour

POSITION SUMMARY:

The Registered Nurse (RN), reporting to the Manager of Hospice Care, will assess, plan, implement and evaluate day-to-day care of residents. The RN will assume a lead role in the interdisciplinary team, providing collaboration, direction and supervision to the Personal Support Worker (PSW) and the Hospice Volunteers. In addition, the RN will liaise with all other health care providers to develop and maintain a resident and family-centred plan of care. While performing all duties of this position, the Registered Nurse will follow the Occupational Health and Safety Act and associated regulations and Hospice of Elgin’s Policies and Procedures.

Key accountabilities of this role include:

Responsibilities

  • Assume the responsibility for all aspects of individual care in accordance with the Regulated Health Professionals Act and the College of Nurses of Ontario Standards.
  • Act as Team Leader on a rotating basis for assigned shifts, which includes determining the assignment of resident care needs and planning the workload, scheduling the breaks for the day in collaboration with the rest of the care team, assisting what any short notice replacements.
  • Clear and concise communication regarding resident status and needs.

Assessment

  • Use evidence-based tools, subjective and objective data, to regularly assess pain and symptom management needs of individuals and families.
  • Recognize, assess, and respond to an individual’s health status changes in accordance with Hospice philosophy.
  • Summarize and report clinical and other situations with other team members, including shift report.

Planning

  • Determine individual care requirements and establish a comprehensive plan of care.
  • Seek medical consultation and direction for a plan of care and pain and symptom control.
  • Consult with the individual, family members and/or the substitute decision maker regarding needs, wishes and preferences for care.
  • Consult with the Ontario Health at Home regarding additional services as required for the care plan.

Implementation

  • Implement and coordinate medical interventions such as medical orders, medications, supplies, and equipment.
  • Ensure communication with other service providers.
  • Administer medication and treatments as required/ordered.
  • Document accurately and in a timely manner according to the College of Nurses of Ontario Standards and Hospice of Elgin policy.
  • Order and maintain medical supplies according to the individual’s needs in collaboration with the Ontario Health at Home Care Coordinator.
  • Identify and report unusual incidents in the delivery of care in the Hospice to the Manager of Hospice Care in a timely manner.
  • Delegate and /or teach skills and techniques to other staff and /or individuals’ families in compliance with the Regulated Health Professionals Act.

Evaluation

  • Review, evaluate and/or revise the Individual’s Care Plan daily.
  • Evaluate nursing and medical interventions.
  • Accurately follow all medication administration practice standards, adhere to Best Practices in the administration of medication.
  • Ensuring the accuracy of the safe medication practices by following the medication management policies of Hospice of Elgin.
  • Confer with individuals and family regarding satisfaction with the plan of care.
  • Assist with the writing of clinical policies and procedures when asked.
  • Participate in the College of Nurses of Ontario Quality Assurance program.
  • Other duties as assigned.

Qualifications

  • Registered Nurse in good standing with the College of Nurses of Ontario.
  • Minimum of 3 years’ experience working in palliative care settings.
  • CAPCE Certificate of Completion or commitment to obtain within 2 years of start date.
  • LEAP Certificate of Completion or commitment to obtain within 2 years of start date.
  • CNA Certification in Hospice Palliative Care preferred.
  • Demonstrated experience providing hospice palliative care, with expertise in pain and symptom management and care provision across all Domains of Issues of Illness and Bereavement.
  • Ability to manage terminally ill individuals and their families, with the necessary knowledge, skill, and physical capacity.
  • Ability to function in a leadership role within an interdisciplinary team environment.
  • Demonstrated critical thinking, assessment, and problem-solving skills.
  • Demonstrated relationship management skills consistent with the Noble Purpose of Hospice of Elgin.
  • Excellent communication and interpersonal skills.
  • Demonstrated computer proficiency.

Additional Requirements

  • Will need to provide proof of vaccination.
  • Satisfactory Vulnerable Sector Records Check.
  • Be available for 12-hour shifts, with a rotating schedule, including weekends and holidays.
