Job Description:

• Establish and maintain links and develop partnerships.
• Act as a resource to, and advocate for individuals, groups, and the community.
• Connects, links, and supports clients through the navigation of the health, social services, and community support systems.
• Provide psychosocial support while guiding clients through system navigation.

• Experience working with marginalized people and vulnerable populations.
• Mental health training an asset.
• Must be a reliable, self-starter committed to connecting people to supports and services.
• Must be confident to work alone and as part of a team.
• Undergraduate degree from a recognized university in a relevant discipline and significant related experience in a community setting.
• Alignment with the Community Health Model of Care and the mission, vision, and values of our Centre.
• Flexibility to work evenings/weekends according to program needs.
• Current vulnerable sector police check required.
